Hair loss treatment by Medifit Biologicals is done in two ways – one is surgical and other is non-surgical (by injections)

1) PRP/PEP (by Injections)
Platelet Enrich Plasma Therapy (PEP THERAPY)
Platelets form an important constituent of blood just like red and white blood cells. They are mainly involved in clotting of blood after any injury. Apart from that they also contain many growth factors(PDGF,TGF,FGF-2,IGF etc.) which play an important role in healing and recovery of the tissues.
PEP has many applications in sports medicine where it is used for recovery of bone, muscle, ligament injuries and presently its use is being tried in many other specialities like neurosurgery, ophthalmology, cardiac surgery, cosmetic surgeries like hair restoration etc.
PEP is autologous and hence immunologically neutral .It is free from allergic and foreign body reactions.
Medifit Biologicals follows gold standard procedure of PRP in specially unit of operations theatres prepared for trichology procedures.
Patient’s blood is withdrawn just as for any other blood investigation. It is then procesed for a fixed period so that cells settle down in layers and plasma is separated. Platelet enrich plasma contains 4-8 times platelets per cubic cm than normal plasma. It is processed further and applied to the scalp with the help of injections or causing slight injury to the scalp.
The rationale for using PEP is to increase the number of platelets and growth factors at the site of tissue injury where it enhances tissue recovery, health. Therapy stimulate the dormant follicular stem cells to produce new hair, increase the diameter of existing hair, prevent or halt ongoing hair loss and increase the blood circulation to that site.
Treatment is given as multiple sessions with interval of about 4 weeks in between two sittings.minimum three sessions required for great results. Patient may be simultaneously prescribed other medications for enhanced effect.
Platelet-derived growth factor (PDGF)—promotes blood vessel growth, cell replication, skin formation
Transforming growth-factor-beta (TGF-b)—promotes growth of matrix between cells, bone metabolism
Vascular endothelial growth factor (VEGF)—promotes blood vessel formation
Epidermal growth factor (EGF)—promotes cell growth and differentiation , blood vessel formation, collagen formation
Fibroblast growth factor-2 (FGF-2)—promotes growth of specialized cells and blood vessel formation; and
Insulin-like growth factor (IGF)—a regulator of normal physiology in nearly every type of cell in the body
2) Hair Transplant (by Local anaesthesia)
Scalp hair actually grows in tiny little groups of 1-4 hairs each. These groups are called “follicular units”. The follicular unit not only consists of a small group of hair follicles but it also contains nerves, blood vessels and a tiny muscle called the erector pilorum. The whole unit is packaged and makes the follicular unit.
Donor area is the area of scalp where the hair for transplantation is taken out for fut or fue .
The hair in this area is dht resistant or permanent in nature.as we observe that even a severly bald person never loose hairs from side and back of scalp.
Recipient sites are the tiny slits or holes that the hair transplant surgeon makes in the patient’s scalp, where the follicular unit grafts are placed during a hair transplant process on the bald area.
MALE PATTERN BALDNESS
Androgenic miniaturization due to dht susceptibility of hair follicle specifically in frontal and vertex part is called male pattern baldness or androgenic alopecia or androgenetic alopecia or sometime alopecia androgenetica .this is most common cause of hair loss in almost 80% male by the age 50 yrs and 40 % female by the age 50yrs.
Classic androgenic hair loss in males begins above the temples and vertex, or calvaria, of the scalp. As it progresses, a rim of hair at the sides and rear of the head remains.
FEMALE PATTERN BALDNESS
The majority of women with androgenetic alopecia develop the female pattern of hair loss. In this pattern, there is no recession of the anterior hairline but a thinning of the hair in the entire frontal scalp which eventually involves the vertex and temporo-parietal area as well. This results in ‘see-through’ hair behind the anterior hairline, that is, the scalp can be seen through the thinned hair over especially the antero-central part of the frontal area.
Determining how many follicular unit grafts will be transplanted in a hair transplant is an important aspect of planning and designing a hair transplant for the maximum possible aesthetic effect. The graft numbers gives general guidelines for the number of follicular unit grafts needed in a patient’s first hair transplant procedure and for subsequent procedures, if more than one session is indicated.
I. Tumescent Anaesthesia
It is an anaesthetic technique which employs large volumes of very dilute local anaesthesia. tumescent formula for scalp surgery is as follows :
Tumescent anaesthesia is commonly used in hair transplant to provide complete diffusion of anaesthetic agent throughout the tissue. This anaesthesia has unique formula of 2% lignocaine 0.9 % sodium chloride solution,8.4 %soda bicarb and 1:10 adrenaline .
The volume that we use is 120 ml nacl ,8 ml lignocaine ,0.8 meq soda bicarb (nahco3 ) and 0.4 ml adrenalin.
So this formula provides a solution that contains 0.1% lignocaine with 1:320000 adrenalin.
Because of large volume of fluid infiltrated in the subcutaneous fat, large fluid pressure is created which diffuse anaesthetic agents throughout the tissue.
Is more effective local anaesthesia, decreased lignocaine toxicity and less bleeding.
Design of anterior hair line
Medifit Biologicals aim of hair transplant is to create a hairline that appears natural and aesthetic. The final appearance should be such that the process is ‘invisible’. A straight and abrupt anterior hairline which does not exist in nature appears contrived and immediately alerts the observer of a previous surgical procedure .experienced transplant surgeons create an irregular anterior hairline which results in a natural and pleasing appearance. Just behind the anterior hairline, the frontal scalp should be densely packed with follicular units, mini- or micro-grafts to create an impression of fullness.
HOW TO SLEEP AFTER HAIR TRANSPLANT ?
Medifit Biologicals recommend our patients use an airplane pillow around their neck after surgery. The airplane pillow is soft enough to minimize irritation and discomfort at the donor site while keeping the grafts safe from any rubbing of the pillow. If the crown was transplanted, an airplane pillow can still be used, but it must be used with caution to minimize any forces that can dislodge the grafts.
We also recommend that my patients sleep in a more upright position (30 degrees or so) to help with the swelling.
